Financial screening is required before an appointment can be made.

In order to qualify, patients must:
Be a resident of Okaloosa or Walton County
Have no medical and/or dental insurance
Have a family income that does not exceed 200% of Federal Poverty Guidelines
Be a U.S. citizen or legal resident
Be an adult (18 to 64 years old)

Documentation required:
Photo ID
Proof of Okaloosa or Walton County residency (utility stub, etc)
Income stubs for the previous 4 weeks for all family members with an income

From the minute you walk into our clinic to when you leave, we hope you feel loved. There are cards for prayer requests in the waiting room, brightly colored pictures in each of the exam rooms, and kind staff willing to listen. Our staff are here to help and give of their time to take good care of you.

Frequently Asked Questions
May IÂ walk-in for a visit?
Unfortunately, no. Our clinic is staffed by volunteers. They let us know when they are available and we schedule appointments in those time slots. Once you qualify as a patient, we would be happy to schedule an appointment for you.
How can I schedule an appointment at the clinic?
If you are already a patient, please just give us a call, otherwise contact us by email or phone to schedule a screening appointment at (850)389-3015.
I can't make it to my scheduled appointment, what should I do?
Please contact us as soon as possible if you know you will not be able to come for an appointment. This allows us to schedule another patient, who needs care, with our volunteers. If you miss 3 appointments, without canceling them at least 24 hours ahead of time, within a 12 month period, you will be dismissed from the clinic program for one year. At that time former patients can be re-screened for clinic services.
How do referrals to other doctors work?
We work with several volunteer specialist in the community. These providers volunteer their time and resources as their schedules allow. Patients are referred on a case-by-case basis.
Does Crossroads care for pain management or prescribe narcotics?
We do not. We realize that we have patients living with pain and we will do our best to connect patients, who have verified diagnoses, to resources in the community who might be able to help but it is a specialty that is outside the scope of this clinic's practice.
Prescription Assistance
Patient assistance programs (PAP) are programs set up by pharmaceutical companies that offer free medicines to individuals who are unable to pay for their medication.
Eligibility varies program by program based on your household income and assets.

Neighborhood Navigator
Neighborhood Navigator is a site that enables people to find help in their area in categories such as food, housing, goods, transportation, health, care, money, education, work, and legal issues.​
Panhandle 2-1-1 offers resources for Substance Abuse and Mental Health needs for our community as well as basic needs such as utility assistance, emergency shelter, and food pantries.
Sharing and Caring
Sharing and Caring coordinates community efforts to meet emergency needs of families in the area. They help find assistance with food, utilities and rent as resources are available.
